Anterior tongue positioning either primary or secondary is recognized in the literature as being associated with open bite.  The Aligner Tongue Trainer Plier was developed (patented, Londos) and used to puncture the elliptical attachments unfilled in the aligner to prevent anterior tongue positioning 800-1000 times/day during swallowing, speaking and eating. This procedure takes less than 20 seconds/tray. Manufactured by RMO®

ATTs or Aligner Tongue Tamers (Trainers,  magnified view), remind the patient to retract the strong, anteriorly positioned tongue (patients report they do not cut the tongue).  In comprehensive Aligner therapy that has one of the highest material costs for both the patient and orthodontist, the 12 ATTs are included and thereby there is every reason to place them into the aligners.

ATTs or Aligner Tongue Trainers indicated from the literature for open bite treatment are shown on the lingual of the lower 3 to 3 region. It is important to note a total of 12 anterior elliptical attachments, upper and lower, are requested in the virtual treatment plan but are left unfilled to be punctured chairside. (Aligner companies will hopefully complete this task in the factory of making the 12 ATTs (patented) for the 20 or more sets of aligners in complex open bite treatment in the future using the Aligner Tongue Star Plier).  ATTs will not function properly without the ellipticals being requested prior to the puncturing because they are flat without the elliptical.
